You care about your work.
You also need something about it to change.
Perhaps you’re questioning how many long-term clients you can keep taking on. You want fewer clinical hours, a different balance of work, or room to teach, facilitate groups, or develop an idea you’ve been carrying.
You may still value much of what you do. And you may be having trouble imagining another decade of doing it this way.
Changing a practice asks you to consider your livelihood, your capacity, and people whose needs matter to you. There are decisions to make, relationships to attend to, and questions about what you want your working life to become.
Together, we’ll clarify what you need, support the decisions you’re facing, and help you navigate the practical and relational work of making a change.
You may know what needs to change before you know how
A practice can be full of meaningful work and still ask more of you than you can keep giving.
Perhaps the schedule that once worked no longer fits your life. Referrals keep coming for work you want to do less of. An idea keeps returning, but there is never room to explore it.
You may have already made a decision several times. Then you imagine telling an actual client, look at the financial implications, or wonder what will fill the space—and begin reconsidering.
These are places where we can work together.
A focused space for a meaningful change
The Practice Shift is five private, 50-minute coaching sessions over eight to ten weeks.
Together, we’ll clarify what you want to keep, work through what needs to change, and begin putting that change into practice. We’ll choose a focus that fits your circumstances and the time we have together.
That might include:
- Reshaping your schedule or reducing clinical hours.
- Changing fees and communicating with existing clients.
- Clarifying the kinds of new work you have capacity to accept.
- Working through a decision that feels complicated by loyalty, responsibility, or others’ expectations.
- Making room for teaching, groups, consultation, or another offering.
- Moving gradually toward a different mix of work.
You don’t need to arrive with a fully formed plan. We can begin with what you know so far.
From considering the change to beginning it
Understand what you need.
We’ll look at what remains meaningful, what has become difficult to sustain, and what you want more room for. Your income needs, energy, and existing commitments belong in this conversation.
Work through the decisions.
We’ll consider your options, the tradeoffs, and what matters most to you. Together, we’ll identify a workable direction and think through the timing, practical implications, and steps involved.
Prepare for the conversations.
We can work on how you want to communicate a change, what needs explaining, and where you find yourself getting stuck. We’ll give attention to the relationships involved and to what you need to express clearly.
Begin and adjust.
As you take the first steps, you can bring the response you weren’t expecting, the decision you’re reconsidering, or the next question that becomes clear once you begin.
We’ll move through this work at a pace that fits your situation. Some decisions need room; others become clearer through action.
Support for the decisions and conversations that feel hardest
Some changes are difficult because you’re unsure what you want. Others are difficult because you know what you need, and you’re worried about what it will mean for someone else.
You may be weighing a fee change, reducing your availability, stepping back from a professional commitment, or reconsidering work people have come to rely on you to do.
We can think through the decision together. What needs your attention? What are the tradeoffs? How do your capacity, livelihood, values, and relationships inform what you choose?
There is room to consider your responsibilities and the limits of what you can continue offering. There is also room for guilt, uncertainty, and the possibility that someone may be disappointed.
When you’re ready to communicate, we can work through that too. In my coaching work, I’ve helped a client work through raising fees with existing clients, email by email, person by person. We can give that same attention to the conversation you’re preparing for—and make space afterward to consider what happened.
You can have company while you decide, while you find the words, and while you navigate what follows.
What’s included
- Five private, 50-minute sessions over eight to ten weeks.
- A shared working plan documenting your chosen change, decisions, and next steps.
- Feedback on up to three short drafts between sessions, such as a client email, practice announcement, or description of a new offering. Each includes one round of feedback within two business days.
- A closing review of what you’ve learned, what is underway, and what comes next.
Questions that need a fuller conversation will become part of our next session.
Our aim is for you to leave with a clearer direction, a workable plan, and the first steps underway. Some changes will continue beyond our time together; we’ll account for that as we plan.

I’m Hilary Kinavey, a therapist, educator, and co-founder of the Center for Body Trust.
Over more than 25 years, my work has grown to include programs, professional training, writing, and coaching. I bring that experience to the practical questions of changing a practice, alongside attention to the emotional and relational questions that come with it.

$1,450 paid in full
or three payments of $484 ($1,452 total)
Five sessions over eight to ten weeks, including the between-session support described above.
Support as you continue
Some changes take longer to carry through. You may be communicating with clients, adjusting your schedule gradually, or developing an offering alongside your existing work.
After The Practice Shift, you can choose Ongoing Implementation Support:
- Two private, 50-minute sessions per month.
- Feedback on up to two short drafts each month, with one round per draft, within two business days.
We’ll work with what is happening as you put your plans into practice—considering responses, making adjustments, and deciding what needs your attention next.
$650 per month. Available after completing The Practice Shift, with no minimum commitment. We’ll check in each month about whether the support continues to be useful.

The Consult is a single 90-minute session for a decision, offer, pricing question, or stuck place you want another set of eyes on.
We’ll look at the practical question and what is making it complicated, with the aim of leaving you with greater clarity and a named next step.
$347 paid in full or two payments of $175 ($350 total).
